Created from video samples of Michael Ashcroft's performance filmed in 1995 in Cardiff, FORM-01, FORM-02, FORM-03 are three interludes realised specially for the technological cabaret Zulu Time by Robert Lepage. It was presented in its final version to the PS1 Clock tower of New York in 2000 under the title FORM.
